Surrogate Decision Making Program
Established in 1986 as an alternative to the court
system, the Surrogate Decision Making Program provides an expeditious
process for obtaining informed consent for individuals with disabilities
lacking a legal guardian or authorized surrogate and are in need of
major medical care.
The counties administered by Common Ground include
the Capital District and the Taconic Region.
